Prosecutors said Jordan Brown covered a 20-bore shotgun, a Christmas present from his father, with a blanket and shot Kenzie Houk in the head before running out of the family's home to catch the school bus.
Her family has claimed that he was jealous of her and her two young daughters. They all lived together in a farm house in Wampum, Pennsylvania, until the shooting last month.
The son Miss Houk was carrying was nearly full term and died of oxygen deprivation. Brown has been charged with murdering him, too.
A judge has ruled that Brown should not be tried as a minor. If convicted as an adult, he faces mandatory life in prison.
Defence lawyers are expected to try to have Brown moved back to a juvenile court.
They said he denied the killing but, being a child, does not fully understand the case against him.
Police said they later discovered a "youth model" shotgun in Brown's bedroom which smelled as if it had been fired recently.
At a preliminary court hearing on Tuesday, police also said that Brown's clothing had gunshot residue on it.
Brown was led into the courtroom in New Castle, Pennsylvania, on Tuesday with his hands cuffed and shackled to a belt on his waist.
Brown is being held at a juvenile detention centre where he is attending school classes, said his lawyer, Dennis Elisco.
"He's OK. He wants to be with his dad. He wants to be home," he said.
